2025社區券申請全攻略：由零開始，手把手教你填表、遞交文件

前言：了解社區券的好處後，最實際的問題就是如何申請。雖然申請過程不算複雜，但當中有些細節值得注意。本攻略將帶你一步步完成申請，確保萬無一失。

第一步：確認資格與獲取申請表格

確保長者已在「安老服務統一評估機制」下完成評估，並正在中央輪候冊上。通常，社署會主動向新登記的合資格長者發出邀請信，信中會附上申請表格。如果沒有收到，也可在社署網站下載或向地區中心查詢。

第二步：填寫申請表格

  • 個人資料：確保姓名、身份證號碼等與官方文件一致
  • 經濟狀況：需要申報長者及同住家人的收入和資產。這是用作計算「共同付款級別」的依據，務必如實填寫

第三步：準備所需證明文件

  • 申請長者的香港身份證
  • 申請長者及同住家人的收入證明（如糧單、银行月結單、稅單等）
  • 住址證明（如水電煤氣單）

第四步：遞交申請

📮 郵寄或親身遞交

至社會福利署社區照顧服務券辦事處

💻 網上申請

透過「長者服務券資訊系統」（VISE）遞交電子申請，方便快捷。

溫馨提示：遞交申請後，社署一般會在2至4星期內完成審批。
