  • 如果長者於2019年3月底或之前已申請加入中央輪候冊，並正輪候社區照顧服務及／或院舍照顧服務；

  • 長者需要在社署安老服務統一評估機制下，被評定為身體機能中度或嚴重缺損，包括身體殘障 (如因中風所引起的活動障礙)和認知失調 (例如因年老出現認知功能減弱或記憶衰退的現象)；

  • 長者並未曾使用院舍服務或社區照顧服務

 

2. 長者社區券的申領流程

與第二階段一樣，要申請第三階段的社區券，家屬需要先到醫務社會服務部、所屬地區的綜合家庭服務中心或長者服務單位提出申請社區券計劃，並請負責工作員為長者作健康及家居護理評估。評估員透過家訪及會面為長者進行評估。

 

被評估為中度或嚴重缺陷的長者，即合資格人士，會收到政府的邀請信參與「長者社區照顧服務券試驗計劃 CCSV」。之後，照顧者與長者需填妥並交回相關表格。長者收到一份「社區券證明書」便可以資助金額購買指定社企、非牟利組織、及私人公司的照顧服務。

7. 選擇服務單位後的程序

社區券持有人選擇認可服務單位後，服務供應商會協助客人選取服務類別、服務組合及確定服務組合價值與應付的共同付款金額。長者可以每月檢定及調整服務模式、服務組合，及服務服務組合價值與應付共同付款金額。

不會。第二階段社區券使用者完成本月服務後，會自動成為第三階段社區券持有人，繼續享用計劃的各項新舊服務。

9. 參加「長者社區照顧服務券試驗計劃 CCSV」會否影響長者在中央輪候冊上的位置？

不會。長者在獲發社區券證明書後，社署只會將其在中央輪候冊上長期護理服務的申請更改為「非活躍個案」處理，並不會把他從輪候冊上剔除。在參與計劃期間，長者如有需要，可透過負責跟進個案的社工，重新啟動其在中央輪候冊上的輪候狀態，將已列為「非活躍個案」更新為「活躍個案」，而其輪候位置會以原本的申請日期為準。
